History
St. Louis Accord spurred development of coalition.
Founded January 2006 with first kick off meeting.
Supported by all CT LTC Stakeholders, including DPH.
Organizational Structure
The coalition's mission is to drive positive changes in LTC from institutional models of care to person-centered environments. The coalition's vision is to promote development of person-centered environments where individuals live, work and grow in nurturing, respectful communities that honor personal choice and diverse lifestyles. The coalition accomplishes this by supporting culture change through education, research, sharing, advocating and collaborative relationships.
Funding
- Working with DPH on CMP funding to support CTCCC activities.
- DPH and CTCCC have collaborated on CMF funded scholarships for CT nursing homes to support enhanced dining experiences and outside gardening activities.
Major Activities This Year
- Working more closely with stakeholders on collaboration to support culture change in Connecticut.
- Continue with quarterly educational programs and general meetings.
- Enhance CTCCC presence in Connecticut and increase membership.
