History
In May of 2005, following the St. Louis Accord, a group of dedicated Minnesota Pioneers came together to: Encourage person-directed models of care; Support educational programs fostering a shared vision; Support and encourage efforts of organizations with shared visions; Advance public policy that furthers the vision; Create opportunities to further the mission of the Coalition within current regulatory requirements

Full coalition meetings are held quarterly with interim committee work. Annual summits have included: Regulation and Culture Change: How They Work Together (provider/regulator summit), Oct. 10, 2006, Bloomington and Partnering for Person-Centered Care (consumer/ policy maker summit), March 23, 2007, St. Paul.



Organizational Structure
Through a 2-year deliberation, the Coalition selected to adopt a stakeholder Accord outlining shared mission, responsibilities, and accountabilities. It is committed to annually using a Member Acknowledgement form. Core to the structure is a third party contracted facilitator, balanced representation of member categories, Convener and Financial Representative positions, and a flexible and working committee structure.

Member organizations involved: AARP Minnesota, Aging Services of Minnesota, Alzheimer's Association, The Board of Examiners for Nursing Home Administrators, Care Providers of Minnesota, ElderCare Rights Alliance, Good Samaritan Society, The Lutheran Home of Belle Plaine, Minnesota Department of Health (survey agency), Neilson Place, the Office of Ombudsman for Long-Term Care, Stratis Health(QIO), University of Minnesota, and Walker Methodist.

Funding

  • Annual dues are established and cover facilitation and meeting expenses ($300-$600 /member). Some members are not required to pay dues.
  • Volunteer staff time and nominal contributions are expected from partners to cover most projects and expenses.
  • Submitted three requests for use of CMP funds for annual summits. Each time the funds (approximately $25,000/event) were used to pay for education of 200-300+ participants.

Major Activities This Year
In 2008, the Accord, the Website and the Resource List were updated. The Membership Acknowledgement form and Dues process were implemented. Building on the success of previous summits, a third half-day summit bringing together providers, all surveyors, state agency staff, ombudsmen and other experts is planned for Oct. 24, 2008 in Blaine. As was a key to the success of previous summits, the facilitation and consultation of Barbara Frank of B&F Consulting will be used in 2008.

The Coalition continues to meet and explore projects that advance the shared vision. Having completed the foundational and organizational goals established by the Coalition's original strategic plan, members will begin exploring the creation of a second multi-year strategic plan. Focus will be given to activities that individual member organizations can't do alone.
